Returning Clients or Clients turning 18

Current active clients: ​To schedule an appointment, please contact your therapist directly. If you need to update your registration or insurance information, please complete the Registration Update form above.

If you are turning 18, please complete the "Appointment Registration Form."

If you are a current client and would like to see a new/different therapist, please complete the "Current Client Internal Referral Form."
