Registration Information

Once you have completed the Appointment Registration online form and the Credit Card Authorization form, our Registration team will reach out to you to match you with one of our therapists. 

It is our goal to schedule you as quickly as possible, as we understand how important it is for you to receive care. Occasionally, there may be delays due to missing information. 

Here are some common concerns and how to resolve them:

Credit Card Authorization
If you completed the Credit Card Authorization form and were not the authorized card holder, we will need you to have the authorized card holder complete a new form on your behalf.

Therapeutic Guest Consent
If you are scheduling family or couples therapy, your partner or guest attending with you will need to complete the Informed Consent for Therapeutic Guests. This form is helpful for explaining the difference between being a client and a guest in therapy.

Our Registration team will send this form to the person you listed on your Appointment Registration form. If you did not provide their information, this may cause a delay as we will need to follow up with you about this.
